What is P.R.I.D.E. ?
Yale University Department of Emergency Medicine is assisting elders and others with impaired mobility through a project known as PRIDE, which stands for Paramedic Referrals for Increased Independence and Decreased Disability in the Elderly.
PRIDE is a community-centered intervention program designed to help anyone who called 9-1-1 because they were unable to get up without assistance and wish to remain safely in their homes, or who enrolled in our program through other pathways. Through this program, paramedics at the time of a 9-1-1- call for a fall, will coordinate closer follow up with visiting nurses, primary care providers, and other community resources to identify and address any unapparent medical conditions and to implement evidence-based falls prevention strategies. Ultimately, PRIDE strives to enhance seniors’ ability to live safely and independently in their own homes, while decreasing their reliance on the 9-1-1 system and hospital emergency departments. PRIDE is a Free, Fast, and Easy way to increase your chances of safely remaining in your home.
It only takes one visit from a PRIDE team member to your home to assess your risk of falling and arrange appointments with health care providers who can help lower your risk of illness and injury at no cost to you.
